CBO Leaders: Share your local insight

We’re inviting Community-Based Organizations (CBOs) across Oregon to share what makes collaboration successful.
 
If your organization provides health-related services such as food, housing, education, safety, transportation, or employment, we want to hear from you!
 
The questionnaire is short, anonymous, and easy to complete. It uses simple multiple-choice questions—no long answers—and covers five key areas: relationships, service delivery, funding, data, and equity. ¡Versión en español disponible pronto!

CHWs: Tell us what’s working in your community

We’re inviting Community Health Workers (CHWs) across Oregon to share their experiences supporting Medicaid members. If eligible, you'll take part in a one-time, 90-minute virtual interview on Zoom to: 
  • Share your experiences helping people access services such as food, housing, safety, transportation, or employment
  • Discuss what makes local partnerships effective and how they can be improved
  • Offer ideas for how collaboration, funding, and data sharing can better support your community
Spots are limited, so sign up early to reserve your interview. Participants will receive a $40 electronic gift card as a thank-you for their time. Participation is voluntary and confidential.

We’re inviting partners and collaborators across Oregon to share this opportunity with the CBOs and CHWs you work with. Sharing this opportunity supports the work you already do to strengthen care coordination, data integration, and resource alignment.
 
By passing it along, you’ll help capture insights that can help reduce unnecessary utilization, close service gaps, and improve population health outcomes through more sustainable care models.
