This study explores how CBOs and CCOs across Oregon work together to improve health and well-being. The goal is to identify the factors that strengthen collaboration and improve outcomes to help regions plan together, align resources across sectors, and invest in community conditions like housing, behavioral health, and education to advance population health.

It is important to identify where strong partnerships are already happening across Oregon and what makes them successful
Support regional planning and implementation strategies for cross-sector collaborations
Learn from CHWs to understand how Medicaid members experience services and what affects their access/quality
It is necessary to understand the real-world experiences that create barriers or drive better outcomes
Enhance local coordination and replicate works well
Analyze data to find out how each local conditions (like indicators of housing,
safety, and environment) are necessary
for partnerships to work well
It is essential to explore how place-based factors impact evidence-based programs, policies, and frameworks
Inform future research and advise local leaders on how to align goals, improve partnerships and address shared priorities
